Medical Transcription Services (MTS) – A Frazier and Associates provides Medical Transcription Services to physicians' offices, clinics, private transcribing services and hospitals. Our transcription team provides timely medical dictation to the highest standards of accuracy, and completeness inclusive of discharge summaries, history and physical examination reports, operative reports, consultation reports, autopsy reports, diagnostic imaging studies and laboratory reports, progress notes, letters, memos and referral letters. Upon completion the transcribed documents are returned to the physicians or other health care professionals who dictated them for review and signature, or correction. These documents eventually become part of patients’ permanent files.
